    "Yooralla works with people with disability to support them to achieve their goals. This may include becoming more independent,being active in community life or managing challenges associated with disability or complex health needs.

    We provide a range of supports to suit an individual’s circumstances, from occasional support coordination to specialist case management support for people with complex health and social needs.

    Coordination of supports

    Coordination of Supports is a service that coordinates and monitors a person’s individual plan including management of their chosen funding administration option.

    Case management

    Case management is a specialised service for individuals with complex needs requiring intensive and comprehensive support. This service provides focused short-term responses to situations of critical need or can manage long-term situations requiring coordination across multiple services.

    Case management helps to build an individual’s capacity (and that of their network) to manage their issues and supports. This service works with the individual through the phases of stabilisation, assessment, planning, implementation, monitoring and closure.

    Case management can also assist people in complex situations to identify issues, plan and implement strategies to support and encourage participation in their community.

    Eligibility

    Yooralla’s Case Management & Coordination of Support services are provided on a fee for service basis. Funding is usually allocated through the Department of Human Services (DHS) or the National Disability Insurance Scheme (NDIS), although some people are self-funded."
